They provide a 24-hours-a-day, seven days a week, service and are ready to respond quickly to provide support during times of illness or if someone experiences an unexpected change in their condition that could develop into a crisis. The teams respond to patients in their own homes. This team is made up of Nurses, Health care assistants, Physiotherapists and Occupational therapists.

Social Prescribers will work with you to find out ‘What matters to you’. The team will then support you in your own community. Complete the self-referral form via the link or ask your GP to refer you to the Sunderland Social Prescribing Service.
