Recovery at Home and Urgent Community Response

They provide a 24-hours-a-day, seven days a week, service and are ready to respond quickly to provide support during times of illness or if someone experiences an unexpected change in their condition that could develop into a crisis. The teams respond to patients in their own homes. This team is made up of Nurses, Health care assistants, Physiotherapists and Occupational therapists.

Sunderland District Nursing and Frailty Nurses Teams

Across the City of Sunderland there are six District Nursing Teams offering a range of services.  Providing holistic nursing care to housebound patients preventing unnecessary hospital admission and allowing patients to be cared for in their own home or chosen environment.

Sunderland Social Prescribing Service

Social Prescribers will work with you to find out ‘What matters to you’. The team will then support you in your own community. Complete the self-referral form via the link or ask your GP to refer you to the Sunderland Social Prescribing Service.
